Student Health Records



Minnesota state law requires schools to maintain a permanent health record for each student. Every child in the district needs to have a health record containing immunizations and health conditions. Health information related to the student's safety and ability to learn is requested upon entrance to the district and needs to be updated annually. Parents are requested to notify the health office at your child's school of any new health problems, updated vaccinations or diagnoses of infectious disease throughout the school year. Health records are considered confidential and shared only with school personnel as needed and with parent permission when required by law. 

**New 2020-2021 school year**  The Annual Health Assessment form is now completed on-line through JMC's annual registration process.
