Emergency Action Plans

If your child has a potentially life threatening or chronic condition (ie: bee sting allergy, asthma, allergy, diabetes, seizure condition,) please fill out an EMERGENCY ACTION PLAN and give it to the health office at your child's school. Please also contact the school nurse before the school year starts and any time you child's condition changes to ensure appropriate changes in health and safety measures are taken.
